We report on a 13 year old boy after bone marrow transplantation (BMT) who received total parenteral nutrition without vitamins. 15 days after BMT he experienced acute life threatening hyperlactic acidemia refractory to bicarbonate and tris. 100 mg thiamine i.v. resulted in a satisfactory clinical and biochemical response.
